This survey, known as the MSTS, is used to assess your health with regards to the limb that has been operated on.  There are separate forms for upper and lower extremities.  If you did not have time to fill this out during your visit, please feel free to fill it out now and email ( This e-mail address is being protected from spam bots, you need JavaScript enabled to view it ) or fax (303-839-6399) it to the clinical research coordinator.

The MSTS measures pain, function, your feelings on the surgery, and three questions specific to the upper or lower extremity.

Each visit (maximum of one per month), we will ask you to fill out one of these to help us see if you have changed since your last visit.
