The Health_Status_Questionnaire (HSQ) is used by our doctors to measure your general health.  Unlike the MSTS, this survey is not specific to one limb but asks questions about your health as a whole picture. 

We only ask you to fill this survey out every six months.  Although it is long, please take the time to answer all of the questions.  If you did not get the time to do it during your office visit, please fill it out online and email ( This e-mail address is being protected from spam bots, you need JavaScript enabled to view it ) or fax (303-839-6399) it to the clinical research coordinator.

If you are a new patient, you will be asked to fill out one of these before your surgery and every six months after your surgery.
