If you have had a lower extremity amputation, we will ask you to fill out this survey.  It measures how well you function with your prosthesis.  If you do not yet have a prosthesis, or are having a new one made, you do not need to answer the questions at this time.

Like our other surveys, we have chosen the LCI-5 because it has been validated and is reliable in literature.  If you have any comments regarding the survey, please feel free to contact the research coordinator either online through this website or during your visit.  We welcome feedback on our outcome measures.
